Specialist palliative care is provided by an interdisciplinary team of specialists whose substantive work is with patients who have a life limiting illness. Specialist care is provided in all settings including community, home, hospital and palliative care units. Organisations that represent or support health professionals working in palliative care or related primary health care services
Clinical management and care coordination including assessment, triage, and referral using a palliative approach for patients with uncomplicated needs associated with a life limiting illness and/or end of life care. Has formal links with a specialist palliative care provider for purposes of referral consultation and access to specialist care as necessary. Organisations that provide education on palliative care to health professionals and/or the general public
Services and agencies that provide support to assist patients with a life limiting illness and their families. Includes homecare services, allied health, meals on wheels and volunteer services.
